Katy Trail Community Health is going to provide school-based dental services in Cole Camp at the school from February 14th through the 24th. To receive these services, parents must fill out a registration packet prior to the service dates. Provided below is a link to the packet where it can be printed. You can complete the form and return it to the school. If you need a paper packet, please contact the school and we will make one available.
